L-ACL

87-year-old Ellen was discharging home and was alone for short periods of time.  Mary, the OT, was concerned if she was going to be safe using the stove top. A cognitive screening called the Allen Cognitive Level Scale was starting to be used by the Occupational Therapy staff.  She had just been trained to do the L-ACL. It was 3 different leather lacing tasks with specific demonstration and verbal directions and having increasingly complex activity demands. She scored 4.4 which indicated she was not safe to use the stove.

             Hmmm! Mary was in doubt on the accuracy of the test. So the next day she gave Ellen a frying pan and asked her to cook an egg like she would at home.  Ellen had trouble sequencing the steps and getting all the necessary food and utensil items.  The biggest safety concerns were she didn’t monitor closely the egg while it was cooking and didn’t turn the stove off afterward.  The family was grateful and per Mary’s safety instructions had an electrician put in a padlocked on/off switch for the stove.

              WOW! Mary became a fan of the L-ACL to screen seniors and also look for confirmations in other self-care tasks, including looking closer at medication management safety.
